Shorehaven Health and Rehabilitation Center is Hiring a Social Services Director Near Oconomowoc, WI

We invite you to consider joining our mission-driven, premier organization in this key role. Celebrating 85 years of service to seniors, Shorehaven Health and Rehabilitation Center is located on the west side of Oconomowoc, on the shores of Lac La Belle. Reporting to our Administrator, the Director of Social Services will lead the Social Services Department to achieve the best care possible for residents and ensure compliance with applicable policies and regulations. Responsibilities will include:

  • Coordinate admissions for long-term care residents
  • Work closely with the Interdisciplinary team to ensure residents are receiving the highest level of services; participate in QAPI team meetings
  • Conduct MDS interviews and record the information; ensure all regulations for social service documentation are met.
  • Provide guidance to department staff; interview and select staff as needed
  • Participate in the development of residents' care plans according to residents’ individual needs and regulations.
  • Assist residents in the completion of the Power of Attorney for Health Care
  • Monitor psychosocial needs, antipsychotic medications, etc., and make referrals to psychiatrists and other professionals, as needed; document progress in meeting identified needs.
  • Assist in the preparation of the annual budget

The ideal candidate will have:

  • Minimum of a Bachelor’s degree in social work or social welfare from an accredited academic institution and be licensed as a Social Worker in Wisconsin, QIDP certification a plus.
  • Minimum of five years’ experience with at least two years in a health care setting
  • Demonstrated skills in organization and leadership.
